Evado Technologies<\/strong> has identified a need in the management of health care for clients (patients) with chronic diseases.\u00a0 Typically, these clients have multiple hospitalisations and multiple chronic diseases.\u00a0 Improving health service delivery to clients in the community will reduce their hospital readmissions and improve their quality of life and life expectancy.\u00a0 These clients usually take a multitude of drugs, have complex care plans, require daily observations, and may need weekly medical or nursing procedures.\u00a0 In many cases, client care is charted on paper, with ad hoc communication among the client\u2019s clinical team members across one or more health service providers.\u00a0 Chronic disease events often result in the client presenting to emergency departments, where staff have little or no knowledge of the client’s ongoing care management and associated charting.<\/p>\n JennieCare<\/strong> enables clients with chronic diseases to receive care at home via a smartphone app or web client, while also allowing their clinical care team to actively monitor their in-home care. JennieCare<\/strong> can continuously monitor the client’s chronic condition by charting care and alerting the clinical team in real time if an issue requires action. The phone app integrates with personal health data collected on the client’s home devices, e.g., Apple\u2019s HealthKit, which collects health data from iPhones and Apple Watches.<\/p>\n JennieCare<\/strong> uses Evado\u00a0<\/strong>Technologies\u2019<\/b> patented record management system to define care plans that specify which health\/medical records to collect to chart the patient\u2019s care. Record layouts can then be used to chart client care and generate relevant medical records. The record layouts can also be pre-coded with relevant medical record codes (e.g., ICD-9 or ICD-10) or a selection of codes when one or more codes may be associated with the medical record.<\/p>\n JennieCare<\/strong> has a comprehensive client privacy system that governs access to client records, allowing the client’s care team to view them and monitor and chart the client’s care over time. All client personally identifiable information is encrypted when stored in the database.<\/p>\n Charted events can be monitored in real time, giving the care team direct access to the latest client records.
