Evado Technologies has identified a need in the management of health care for patients with chronic diseases. Typically, these patients have multiple hospitalisations and multiple chronic diseases. Improving health service delivery to home-based patients will reduce their hospital readmissions, improve their quality of life and life expectancy. These patients usually take a multitude of drugs. They have complex care plans, require daily observations, and may need weekly medical or nursing procedures. In many cases, patient care is charted on paper, with ad hoc communication among the patient’s clinical team members across one or more health service providers. Chronic disease events often result in the patient presenting to emergency departments, where staff have little or no knowledge of the patient’s ongoing care management and associated charting.
JennieCare enables patients with chronic diseases to receive care in their homes via a smartphone app or web client, while also allowing their clinical care team to actively monitor their in-home care. JennieCarev can continuously monitor the patient’s chronic condition by recording care and alerting the clinical team in real time if an issue requires action. The phone app integrates with personal health data collected on the patient’s home devices, e.g., Apple’s HealthKit, which collects health data from iPhones and Apple Watch.
JennieCare uses Evado Technologies’s patented record management system to define the health/medical records to be collected to chart the patient’s care. Record layouts can then be used to chart patient care and generate relevant medical records. The record layouts can also be pre-coded with relevant medical record codes (e.g., ICD-9 or ICD-10) or a selection of codes when one or more codes may be associated with the medical record.
JennieCare has a comprehensive patient privacy system that governs access to patient records, allowing the patient’s care team to view them and monitor and chart the patient’s care over time. All patient personal identifiable information is encrypted when stored in the database.
Patient-charted events can be monitored in real time, giving the care team direct access to the latest patient records. If the patient presents to the ED, ED clinicians will have direct access to the patient’s in-home care records, providing a clearer view of the patient’s observational records prior to presentation.
When fully developed, JennieCare will be able to integrate with other health IT systems and My Health Record, enabling patient records to be shared with relevant health service providers.
