Evado Technologies has identified a need in healthcare management for patients (clients) with chronic diseases. Typically, these patients have multiple hospitalisations and multiple chronic diseases. Improving health service delivery to community patients will reduce hospital readmissions and improve their quality of life and life expectancy. These patients usually take a multitude of drugs, have complex care plans, require daily observations, and may need weekly medical or nursing procedures. In many cases, client care is recorded on paper, with ad hoc communication among the client’s clinical team members across one or more health service providers. Chronic disease events often result in the patient presenting to emergency departments, where staff have little or no knowledge of the client’s ongoing care management and associated charting.
JennieCare enables patients with chronic diseases to receive care at home via a smartphone app or web client, while also allowing their clinical care team to actively monitor their in-home care. JennieCare can continuously monitor the client’s chronic condition by charting care and alerting the clinical team in real time if an issue requires action. The app integrates with personal health data collected on the client’s home devices, e.g., Apple’s HealthKit, which collects health data from iPhones and Apple Watches.
JennieCare uses Evado Technologies’ patented record management system to define care plans that specify which health and medical records to collect to chart the patient’s care. Record layouts can then be used to chart client care and generate relevant medical records.
JennieCare has a comprehensive client privacy system that governs access to client records, allowing the client’s care team to view them and monitor and chart the client’s care over time. All client personally identifiable information is encrypted when stored in the database.
The care team can monitor charted events in real time, giving them direct access to the latest client records. If the client presents to the ED, ED clinicians can access the client’s charted care records, providing a clearer view of the client’s observational records prior to presentation.
When fully implemented, JennieCare will fully integrate with eNRMC and eMAR medication management systems, My Health Record, personal health devices, and HL7 hospital systems, enabling patient records to be shared with relevant health service providers.
